How to register
We welcome requests for registration from patients living in or moving to the practice area (see attached boundary map). PRACTICE BOUNDARY.doc You can register by bringing along your NHS medical card or completing a form available from reception.
To save time you can also download and print a registration form. You will need to complete this by hand and bring it to the surgery.
We recommend that new patients undertake a health check with a practice nurse. You will need to complete a New Patient Questionaire to ensure that your records are accurate. New Patient questionnaire.DOC
Clinics
We do not operate specific clinics for our nursing care.
To ensure that you are seen by the appropriate nurse, please inform the receptionist of the reason for your appointment when booking.

Sick certificates
You only need a doctor’s note if you are unable to work and are ill for longer than seven calendar days. Your employer will provide you with an SC2 self-certificate form for shorter periods of illness. If your employer will not accept your self certificate form, a private medical form can be provided for a small fee (see non NHS Services below).
Test results
Test results are normally returned within 2 weeks.
Since our telephones are very busy in the early morning, we ask you to telephone for test results (eg blood, urine, x-ray) between 11 and 12.30 each day. Please be aware that reception staff are not medically qualified and cannot discuss results. If you have any queries, you must speak to a doctor or nurse about any test result.
To ensure confidentiality, we only release results to the patient, unless alternative arrangements have been agreed in writing.
Non-NHS services
The NHS does not pay for some of the services we provide. These include private sick notes, insurance forms, holiday cancellation forms, medical reports, fitness to travel certificates, private prescriptions, passport signing and some vaccination services. Our fees for these services, which are in line with BMA approved national guidelines, are attached.
Please note that NHS services take priority over non-NHS services and you should therefore allow 1-2 weeks for completion of this work.
