Repeat Prescriptions


Repeat prescriptions may be issued for regular medication as arranged by your doctor.

 

In order to provide a safe and accurate service we would request that orders for repeat prescriptions are made

-    in person by leaving your completed repeat prescription request slip in the prescription box in reception, or

-    by post, enclosing a stamped addressed envelope with your request form, or -

-    by FAX to 01332 720066 (Chellaston), or 01332 865154 (Melbourne)

-    Online requests - To access on-line services you must first request a username & password by completing the attached registration form & returning it to our reception staff. registrationform .   Once you have your username & password, click here to order repeat prescriptions online.

 

Please note that we require 48 weekday hours  to process your request for repeat medication whether it is ordered in person, by leaving your request slip, online, by post, by fax or by telephone.

 

If you are housebound and unable to use the above methods, then requests for repeat prescriptions may be made by telephone between 9.00am to 10.00am on 01332 720088 (Chellaston Medical Centre)  and  01332 862341 (Melbourne Medical Centre). 

Prescription requests by telephone will only be taken from the above numbers, as reception staff are unable to process requests.

Please avoid telephoning on Mondays for routine prescriptions as the telephone lines are extremely busy.

Please plan ahead when ordering your prescription so that you do not run out of your medication.

If you have your prescription dispensed by a regular pharmacist, the pharmacist may be able to collect your prescription on your behalf. Please ask our receptionist for details.

You will be required to see a GP for a medication review after a certain number of repeat prescriptions.
