Medical records are kept confidential & secure.
Information is only available to those involved in your care and may be used for audit purposes.
You have a right to see your records and should put your request in writing if you wish to have access. Online access can be arranged. If you require a paper copy of your records this can take up to 1mth to provide.
If you change your name, address or telephone number, please let our receptionists know by calling or writing to us. If you move outside the practice area you will need to find a doctor in your new area. The Receptionist will advise you if this is the case.
